Transition planning for young adults who are deafblind is an important part of setting and reaching individualized goals and should begin as early as 14 years of age and continue up until the student leaves school. Transition is most successful when there is a team approach that develops through home, school, and community collaboration.

The Virginia Deafblind Project can support the team through a range of technical assistance including:

  • Customized on-site and virtual coaching on deafblind specific strategies including transition planning using the Discovering ME! Transition Assessment Process and Person Centered Planning.
  • Family support from the Project’s Family Engagement Coordinator.
  • Virginia Deafblind Transition Retreat: An annual event that brings together deafblind young adults and mentors across Virginia to address different aspects of transition planning and self-determination.
  • Facilitating collaboration with IEP team and Rehabilitative Services.
